A really bad ear is bright red almost bleeding with lots of dark wax and or yellow pus. When there's yellow pus you start thinking about a "weird germ" and cultures. But not always.
With a "bad ear" #badear it's primarily an issue of
Cleaning the ear out
Reducing inflammation in the ear
Possible pain meds
Killing germs, yeasts and even parasites (if suspected) in the ear.
A few days after the above --> If everything's a bit better you can really examine the ear to the ear drum without making the dog want to die. And without as much wax and goo in the way, you can actually see stuff.
The recheck is critically important.
